- From SR 60: Take US 98 / 17 North (in Bartow), stay on 98 North, clinic on left just before Polk Parkway 570. Enter on Winter Lake Road.
- From US 17 South: Take 17 North to Bartow, then take 98 North. Clinic on left before Polk Parkway 570. Enter on Winter Lake Road.
- From US 17 North: Take 17 South, turn right on SR 540 – at intersection with SR 98, go straight (becomes Winter Lake Road). Clinic is on right.
